The guy cheated on you SEVERAL times and you took him back. That speaks volumes. A person who is EARNESTLY SORRY the first time around does his very best NEVER to do it again. He will avoid any compromising situations and temptations because of his committment to you. This didn't happen here. The second time means that the problem is HABITUAL, there is no remorse, and the person needs to be CUT OFF! It doesn't matter how long you've been together. If it's UNHEALTHY, you don't need it in your life! Stop treating this like a drug addiction!
